Please do not use this form if you have an urgent medical problem or need to reschedule an existing appointment. Instead, contact our office at (937) 398-1066 .
Established patients may enroll and log into the patient portal (see patient portal button below) to request appointments, view health records, view statements, pay a bill, send messages to the office, and update your health information.
New to Practice?
Select the Pre Register Button on the patient portal home page. Once your Pre Registration is complete, you can click ENROLL NOW. Once enrolled, you can request a new appointment and a member of our staff will call you to schedule within 48 hours.
